I See Me Books. I have ordered several for nieces, nephews etc and both of my kids have one. I have been very happy with them. The price varies but I think around $35. I always try to order when they have free shipping.

I actually just received one in the mail that I got for my oldest. It was a Sweet Deal with Flattenme.com. It turned out great! The quality is really nice and they did an amazing job putting her head in it (your child is the star of the book). I ended up paying a little more to get the hard cover and I'm glad I did. I figure it's something she'll have forever so I wanted it to last. I think full price they're around $30-40 I don't remember. The only sort of negative thing I can say is that I didn't love any of the stories. I just thought they were kind of ok.
