Try buying a peice of particle board at Lowes and putting it over the slats to give the rails some stability.
I just bought my 2 yr old son a twin bed, but it has no box spring and only slats (no hard surface) under the mattress. He is quite a mover, and will surely fall out of the bed without a rail, but I cannot find a rail that will be secure without a box spring or a hard surface beneath the bed. Any suggestions?

We tried the removeable bedrails by Safety First and they were OK. In the long run, our little guy didn't fall out of bed much even though he is quite a mover. YOu could even try a couple of lankets on the floor or pillows to cushion the fall. We did that as well. He just had to be careful not to trip when he got out of bed to go potty at night.
When my son was younger and I wanted his bed closer to the floor, we just used a sheet of plywood instead of a boxspring, actually I think it was two separate sheets that fit together to fill the space perfectly. He had a full size mattress and we used one of those net type rails that has a metal part that slides under the mattress, if that makes any sense, so that he wouldn't fall out of bed.

I bought a bunky board to put on the slats. You can find this at any mattress store. The rails should then fit on top of the board.
I asked a similar question a few weeks ago. A couple of people suggested using a swimming pool noodle. They said you can cut it to fit the length of the mattress and then tuck under the fitted sheet at the edge. It should make just enough of a bump to keep your child from falling out of bed. I haven't tried it yet with my toddler (haven't moved her to the toddler bed yet), but I am planning to try this when we do move her.

My daughters bed only has slats underneath and we don't have a box spring for her either. We have been using a bed rail similar to this from Babies R Us for nearly a year and it is stable and works great. It is also portable and ours folds down under the mattress but we don't use that feature.

You can try getting a bunkie board. It's a flat board that replaces the box spring in beds like you are talking about. They are about 2" deep. That would give you a flat surface so the bedrail will work without raising the bed any like a box spring would. You can get them at any mattress store and maybe even check Grace or Goodwill.
